Home Purchase (or) Refinance Documentation Checklist

  • 30 Days Pay Stubs *all borrowers
  • Most recent 2 years W2's and/or 1099's
  • Most recent 2 years Federal Tax Returns*
  • 2 months bank statements from at least 2 accounts (Checking, Savings, etc.) *all pages
  • Two forms of Identification (Driver’s License, Passport, Social Security Card)
  • Fully Executed Purchase Contract *if applicable

If you are applying for a refinance loan, we will also need your most recent mortgage statement and a copy of your homeowner's insurance policy (Declaration Page).

Upon completion of your secure online application, you may upload the above documents into your secure application portal, e-mail to (or) fax documents to 800-647-8777.

*3 years of Federal Tax Returns will also be required if you are also applying for an MWF approved Down Payment Assistance Program.
